Coupons are removed from the store's inventory when they're purchased, not when they're added to a cart. A byproduct of this behavior is that it's possible to put something in your cart and then for it to be gone when you go to checkout. This is most likely to happen after we email our customers soon after issuing new coupons.
Most stores on the internet behave this way because it's the lesser of 2 evils. If inventory was adjusted when something was added to a cart, it would allow someone to maliciously take down the site just by adding a large number of coupons to their cart. Then there's the issue of abandoned carts where folks add coupons but never checkout. From a technical standpoint, there's no answer to how long the store should wait before determining that a cart was abandoned so that it can add the items back to inventory.
The good news is that we have a very large inventory of inserts and that we continuously process them, so if you can't get a coupon today you'll almost always be able to get it soon thereafter.
